01Testosterone Optimization 101

The procedure itself.

Testosterone optimization restores testosterone to physiological levels in patients with documented deficiency. It can be delivered via injection, topical gel, or pellet implant. Most commonly used in men, with selected indications in women.

How long it's been around
Testosterone has been used clinically since the mid-20th century. Modern prescribing is risk-stratified and requires documented biochemical deficiency.
What we treat with it
In men: documented hypogonadism causing low libido, erectile dysfunction, fatigue, muscle loss, mood changes. In women (selected cases): low libido despite adequate oestrogen, surgical menopause with persistent symptoms.

Testosterone optimization disappoints when prescribed without documented deficiency, when monitoring is neglected, or when expectations are set against unrealistic anti-aging marketing.

Testosterone deficiency is a biochemical diagnosis — not just a symptom complex. Baseline labs required: total and free testosterone (morning, fasting), CBC, PSA (men over 40), lipid panel, fasting glucose.

How is testosterone deficiency diagnosed?
By morning, fasting total testosterone (and free testosterone). A single low result is not enough — testing should be repeated to confirm.
Will testosterone make me infertile?
In men, exogenous testosterone suppresses natural production and spermatogenesis. Men planning future pregnancies should defer treatment or use alternative strategies.
What delivery method is best?
Topical gels, intramuscular injections, and pellet implants all work. Choice depends on convenience, dose stability needs, and patient preference.
How long until I feel the effects?
Libido often within 2–4 weeks; energy and mood 4–8 weeks.
Will testosterone cause prostate cancer?
Current evidence does not show testosterone replacement causes prostate cancer. However, testosterone can accelerate growth of any existing cancer, which is why screening before and during therapy is part of the protocol in men.
