Shockwave therapy uses focused or radial pressure waves applied externally to stimulate tissue regeneration. In gynaecology and sexual wellness it is used for selected indications including erectile dysfunction and Peyronie's disease.
How long it's been around
ESWT has been used in urology and orthopaedics since the 1980s. Sexual-wellness applications have grown in the past decade.
What we treat with it
Vasculogenic erectile dysfunction (men), Peyronie's disease (selected), chronic pelvic pain (selected indications). Evidence base varies by indication.
The honest version
Why shockwave therapy sometimes disappoints.
Shockwave disappoints when expectations are set against thin evidence, when the wrong indication is chosen, or when underlying conditions weren't addressed.
The evidence base for shockwave in sexual wellness is mixed. For vasculogenic ED the evidence is moderate; for many other commonly-marketed indications the evidence is thinner than marketing suggests.
Erectile dysfunction can have hormonal, vascular, psychogenic, or medication-related components. Treating with shockwave without addressing underlying contributors produces underwhelming results.
